Anthem MediBlue Medicare Plans 2016 – Connecticut

Anthem will be offering three Anthem MediBlue Medicare Plans in Connecticut for 2016.

MediBlue Dual Advantage (HMO SNP)

will be available in all 8 Connecticut counties. This is not only a $0 premium plan but also has, an in-network out-of-pocket maximum of $6,700. Plan offers $0 co-pays for most services including 2 oral exams and 1 dental x-ray. This plan also offers one routine hearing exam an a maximum hearing aid benefit of $1,500. One eye exam and a $125 eye glass/contact benefit is also provided.

MediBlue Select (HMO)

is available in Hartford County only. It offers a $26 monthly premium, and also an in-network out-of-pocket maximum of $6,100. Participants pay $395 for days 1-4 and $0 for days 5-90 for in-network inpatient hospital stays. Primary care physician co-pay is $5. Specialist co-pay is $40. Outpatient lab co-pay is $10. X-ray co-pays range form $50 to $70. ER co-pay is $75. Urgent care co-pay is $55 and ambulance co-pay is $275. The MediBlue Select plan includes one routine eye exam per year.

Not only offers a Part D deductible of $220. But also, preferred generic co-pays are $4 for 30 days preferred, $9 for 30 days standard and $12 90 days mail order. Generic co-pays are $10 for 30 days preferred, $15 for 30 days standard and $30 90 days mail order. Preferred brand co-pays are $42 for 30 days preferred, $47 for 30 days standard and $126 90 days mail order. Non-preferred brand co-pays are $95 for 30 days preferred, $100 for 30 days standard and $285 90 days mail order. Specialty Tier drug co-pays are 28% for preferred (mail order not available).

MediBlue Plus (HMO)

Plan is available in not only, Fairfield, Litchfield and New Haven but Windham Counties as well. It offers a $37 monthly premium with an in-network out-of-pocket maximum of $6,700. Participants pay $395 for days 1-4 and $0 for days 5-90 for in-network inpatient hospital stays. Primary care physician co-pay is $15. Specialist co-pay is $40. Outpatient lab co-pay is $25. X-ray co-pays range form $65 to $200. ER co-pay is $75. Urgent care co-pay is $60 and ambulance co-pay is $300. The MediBlue Plus (HMO) includes one routine eye exam per year.
